Indications/Uses
Management of HTN & prophylaxis of chronic stable angina pectoris.
Dosage/Direction for Use
Adult HTN Initially 5 mg once daily. Usual maintenance dose: 2.5-10 mg once daily. Standard maintenance dose w/ other antihypertensive agent: 5-10 mg once daily. Stable angina pectoris Initially 5 mg once daily, may be increased to 10 mg once daily. Elderly, hepatic impairment Initially 2.5 mg once daily.
Administration
May be taken with or without food: Take tab in the morning. May be taken w/o food or w/ a light meal not rich in fat or carbohydrate. Swallow whole, do not divide/chew/crush. Avoid grapefruit juice.
Contraindications
Hypersensitivity to felodipine & other dihydropyridines. Discontinue in patients who develop cardiogenic shock. Unstable angina pectoris. Patients w/ clinically significant aortic stenosis, haemodynamically significant cardiac valvular obstruction, dynamic cardiac outflow obstruction, uncompensated heart failure & during or w/in 1 mth of acute MI. Pregnancy.
Special Precautions
Not suitable for use as acute therapy in hypertensive urgencies or emergencies. Significant hypotension w/ subsequent tachycardia leading to MI in susceptible patients; mild gingival enlargement w/ pronounced gingivitis/periodontitis. Propensity for tachycardia; secondary prevention of MI; malignant HTN; severe left ventricular dysfunction. Hereditary galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Concomitant use w/ strong CYP3A4 inducers or inhibitors. May affect ability to drive & use machines. Reduced liver function. Not to be given during pregnancy. Not recommended during lactation. Not recommended in childn.
Adverse Reactions
Drug Interactions
Increased plasma conc w/ quinidine, diltiazem; enzyme inhibitors eg, verapamil, cimetidine, erythromycin, itraconazole, ketoconazole, HIV PIs, NNRTIs, quinupristin, dalfopristin, ethanol, nefazodone, grapefruit juice. Increased tacrolimus exposure. Decreased plasma conc w/ enzyme inducers eg, phenytoin, carbamazepine, phenobarb, rifampicin, barbiturates, efavirenz, nevirapine, St. John's wort. Increased risk of severe hypotension, exacerbation of angina, CHF & arrhythmia w/ β-adrenergic blockers eg, propranolol, timolol. Severe hypotension during surgery w/ fentanyl. Increased serum digoxin conc. Increased incidence of severe hypotension w/ methyldopa, hydralazine, captopril, doxazosin.
